NBGA Incident Report Form
2010-04-01Please find below a link to the official NBGA Incident Report Form. This form needs to be filled out and sent to NBGA within 3 days of an injury (all injuries) as there is a limited time to submit these to the insurance company in order to be eligible for any potential claims.
